Coordinated Care Conferences
Real progress happens when everyone is on the same page. We bring families, schools, and providers together for regular care conferences — keeping every child’s plan aligned across Anoka, Hennepin, Ramsey, Washington, Scott, Dakota and Carver counties.

What Are Care Conferences?
One shared plan, updated together
Children receiving EIDBI services often work with multiple people — therapists, school staff, care coordinators, and family members. Coordinated Care Conferences bring everyone together on a regular basis to review progress, align on goals, and make sure no part of your child’s care falls through the cracks.
