Coordinated Care Conferences

Real progress happens when everyone is on the same page. We bring families, schools, and providers together for regular care conferences — keeping every child’s plan aligned across Anoka, Hennepin, Ramsey, Washington, Scott, Dakota and Carver counties.
What Are Care Conferences?

One shared plan, updated together

Children receiving EIDBI services often work with multiple people — therapists, school staff, care coordinators, and family members. Coordinated Care Conferences bring everyone together on a regular basis to review progress, align on goals, and make sure no part of your child’s care falls through the cracks.

Why It Matters

Coordinated care means fewer gaps

You get one clear picture of your child’s progress, instead of piecing together updates from different sources
School staff and therapists stay aligned on strategies, rather than working in isolation
Concerns get addressed early, before they become bigger issues
Your family has a consistent, scheduled opportunity to ask questions and voice concerns
